On August 21, 2024, Pomona Community Health Center d/b/a ParkTree Community Health Center (“ParkTree” or “PCHC”) filed a data breach notification with the California Attorney General after determining that an unauthorized party was able to access information stored on the company’s computer network. In this notification, ParkTree explains that the incident resulted in an unauthorized party being able to access consumers’ confidential information, including first and last names, dates of birth, health insurance identification numbers, medical record numbers, contact information, gender, and medical information. After completing its investigation, ParkTree began sending data breach notification letters to all individuals whose information was affected by the recent data breach.

What caused the data breach at ParkTree Community Health Center?
The data breach at ParkTree Community Health Center was only recently disclosed and more information is expected in the near future. According to these sources, PCHC experienced a network outage on February 7, 2024. In response, PCHC launched an investigation to learn more about the incident and determine if confidential patient information may have been affected. Through this investigation, PCHC learned that an unauthorized party was able to access files containing confidential patient information on February 7, 2024.
After learning that confidential consumer data was accessible to an unauthorized party, ParkTree Community Health Center reviewed the compromised files to determine what information was leaked and which consumers were affected. PCHC completed this process on July 9, 2024. While the compromised information varies by individual, it may include your first and last name, date of birth, health insurance identification number, medical record number, contact information, gender, and medical information.
On August 21, 2024, ParkTree Community Health Center sent out data breach notification letters to everyone affected by the recent data breach. These letters were intended to provide victims with a list of the information belonging to them that had been compromised.
